Definitely, at this time of year, when we have all had a long, sunless winter, fatigue can be at its worst. Diet can play a major part in influencing how tired or energized you feel. Fatty liver how to fix NAFLD

Fatty Liver Did you know Fatty liver affects 1 in 3 of us and if you have diabetes this figure is higher again. Scarily the end result, if not addressed, can be a decompensated liver and even liver failure. So, if you have been thinking about losing weight and getting more active but really don’t feel the need, you may wish to think again. Many of us bury our heads in the sand and believe we are getting along fine.  No aches no pains, enjoying our current way of eating, drinking and moving when we have to. So why change? Well, today, the most common cause of chronic liver disease is nonalcoholic fatty liver disease (NAFLD) AND…. 95% of people with NAFLD do not know they have it. I have many patients come to see me for weight loss, diabetes or healthy eating advice, and when we investigate their health further, we uncover blood markers that don’t read well. It is such a common problem and one that I fix daily so I feel it is important for you to be aware. I have been researching some information to help you further, and in this blog I have pulled a few interesting details from medical papers which I have added in below. So let’s dive in. What is non-alcoholic fatty liver disease NAFLD? It is a liver disease that is NOT caused by alcohol use. Characterized by fatty infiltration of the liver. What is the main cause of a fatty liver? Fat builds up in the liver by eating excess calories. This excess energy is too much for the liver to process and too much fat creates insulin resistance and can lead to inflammation. Fatty liver can occur if you have other conditions, such as obesity, diabetes, high triglycerides, and metabolic syndrome. What are the signs of a fatty liver? There are usually NO signs of fatty liver disease in the early stages. But you may feel fatigued and have pain in the right upper quadrant  (upper right part of the abdomen)- it’s usually a dull discomfort and you may wish to investigate. How serious is a fatty liver? Early-stage NAFLD, if sorted early enough thankfully can be reversed, and doesn’t usually cause harm. But if not treated it can lead to serious liver damage, including cirrhosis. Having high levels of fat in your liver is also associated with an increased risk of problems such as diabetes, heart attacks, and strokes. What is liver cirrhosis? Cirrhosis is scarring (fibrosis) of the liver caused by long-term liver damage. The scar tissue prevents the liver from working properly. If cirrhosis (the most advanced stage) occurs, you can get more severe symptoms, such as yellowing of the skin and the whites of the eyes (jaundice), swelling in the legs, ankles, feet, or tummy (oedema) and itchy skin. How do you fix a fatty liver? The good news is that fatty liver disease can be reversed—and even cured—if patients take action early, including a minimum 10% sustained loss in body weight and/or a change in the foods you eat. How long does it take for fatty liver to go away? An average period of 6 weeks to 2 months is an expected timeframe to recover from fatty liver disease. However, lifelong adherence to particular diet and lifestyle changes may be necessary to prevent relapse. What foods are good for fatty liver? It has been known for many years that Vitamin D is important for the following reasons: Vitamin D helps our body absorb calcium and phosphate, lowering the risk of tooth decay, cavities, and gum disease as well as ensuring our bones are flexible and strong. Vitamin D support the immune system which allows our body to fight off harmful bacteria and viruses. Vitamin D helps to strengthen muscles. Excitingly there is more to vitamin D than it was originally believed…..but before we get into that lets cover the basics…. 2020 with COVID 19 – How our family recovered…

2020 was a tough year for the world with the COVID 19 pandemic and 2021 continues to see no end to this virus. The vaccinations can hopefully help reduce hospital admissions however we still need to keep others safe. Reflecting over the past year we have had our ups and downs but over all I am one of the lucky ones and wake everyday thankful for what we have. We are fortunate that my husband has a secure job and really glad my kids are easy to manage as teens. We are fortunate we can sleep and eat in the safety and comfort of our warm home and we have a large park beside us to walk our dog. I am also very fortunate to be have my Dietetic nutritional consultancy business www.121dietitian.com online to continue to help and advise others. I am passionate about wholefood nutrition and this really matters with our current pandemic. Not being able to visit loved ones has been very difficult. I am especially sad about not seeing my parents. My mum has Multiple Sclerosis and dependent on hoists and wheelchair and loves company. She needs many to assist her daily needs. Sadly they are 120 miles or a 3 hour car drive away in a different country – not easy to visit, so in between the lockdowns we have hopped in the car driven and spent a few hours chatting in the window to them returning home relieved they are safe and coping. Summer 2020 was a tough time for my eldest 2 children: A-levels and GCSE’s were cancelled and they were hugely disappointed. They are now doing their best to continue focused for the future but it is damn hard. I am very proud of them. Fast forward December 20 everything was going well, life was in a routine with work and school and we were preparing for Christmas at home. We managed to enjoy Christmas day and then COVID struck (brought home from my husbands work). We began to fall ill first my husband and then me. We told no one as we were scared of what was ahead. We self isolated and were fortunate to have extra food in with Christmas. The kids also had varying symptoms. I can’t say for sure but as I write this we have got away lightly. Loss of taste and smell and fatigue being the most difficult for 2 weeks. We were dreading the worst as both myself and my husband are healthcare professionals and have seen first hand what COVID 19 is doing to people. The longer term damage of kidneys, heart, brain, stroke and lungs is all too real. I have over the past year of COVID been advising others on assisting their immunity and improving their overall health. Little did I think we would be saved by it too. I personally feel that having underlying good health, eating a varied diet, taking Vitamin D, getting restorative sleep and keeping a safe alcohol intake has been advantageous in our recovery. 2021 has been a slow start for us but when you have your health you have your wealth so it can only hopefully get better. Essential Nutrition for Recovery after Surgery (Part 2) In this follow up to our essential Nutrition & Surgery focus part one, Director and Principal Dietitian Gillian Killiner, continues in this blog to discuss the key role that vitamins and minerals play in helping your body recover after surgery. Vitamin A – essential for recovery after surgery Vitamin A is commonly known as the anti-infective vitamin for surgery. It is central to normal functioning of your immune system. Vitamin A is also needed to maintain the integrity and function of your skin and mucosal cells. When you enter a hospital or clinic for surgery, many things will stress you. You will also be exposed to bacteria and viruses that your system is not used to. Infection will quickly use up your vitamin A stores. In this way, infection starts a vicious cycle, because not enough vitamin A is related to increased severity and likelihood of death from infectious disease. It is important to go into surgery with a good supply of vitamin A in your tissues. You will need to continue consuming enough vitamin A to keep those levels optimal. Vitamin A rich foods include: beef liver, carrots, sweet potato, kale, romaine lettuce, apricots, broccoli, butter, eggs, cantaloupe melon and red pepper. Vitamin E essential nutrition for recovery after surgery The main function of vitamin E in humans is as an antioxidant which are known to neutralizes toxic radicals in the cells. Vitamin E has been shown to improve immune system functions that decline as people age. It helps increase blood flow. It does this by preventing blood clots and relaxing blood vessel walls. These properties all shorten post surgery recovery time. Vitamin E rich foods include: sunflower seeds, almonds, hazelnuts, mango, avocado, butternut squash, broccoli, spinach, kiwi and tomatoes. Vitamin D – essential for recovery after surgery Vitamin D is a fat-soluble vitamin needed for normal calcium metabolism. You make vitamin D in your skin when you are in the sun without clothing or sunscreen. Vitamin D regulates your immune system function during times of stress. Adequate vitamin D levels are important for decreasing the risk of high blood pressure. If you do not have enough vitamin D you can prolong your recovery time. With deficiency you will not absorb enough calcium. Then your body will steal calcium from your bones. This will increase complications and longer term your risk of osteoporosis and other health problems. Vitamin D deficiency causes muscle weakness and pain. Obesity increases the risk of vitamin D deficiency. Once vitamin D is made in the skin or ingested, it is deposited in body fat stores. Storage makes it less available especially to people with large amounts of body fat. Vitamin D rich foods include: halibut, mackerel, salmon, rainbow trout, cod liver oil, sardines, tuna and eggs. Iron – essential nutrient for recovery after surgery Iron is required for a number of vital functions, including growth, reproduction, healing, and immune function. You need the right amount of iron for hundreds of proteins and enzymes. Post-operative anaemia is associated with poorer surgical outcomes including infections, increased length of hospital stay, circulatory overload, and death. Iron rich foods include: spirulina, liver, beef, lentils, dark chocolate, spinach, sardines, black beans, pistachios and raisins. Copper – essential for recovery after surgery You must have enough copper in your system for normal iron metabolism and red blood cell formation. Anaemia is a sign of copper deficiency. Copper is required for you to be able to move iron to your bone marrow for red blood cell formation. Vitamin A deficiency will make iron deficiency anaemia worse. Taking a combination of vitamin A and iron will protect you from anaemia better than either iron or vitamin A alone. Copper rich foods include: beef liver, shitake mushrooms, cashews, chickpeas, kale, cocoa powder, sesame seeds, quinoa, almonds, lentils and chia seeds. Calcium essential nutrition for surgical recovery Calcium is the most common mineral in the human body. About 99 percent of the calcium in your body is found in your bones and teeth. The other one percent is found in your blood and soft tissue. The functions of calcium are so vital to survival that the body will steal calcium from your bones. It does this to keep blood calcium levels normal when your calcium intake is too low. Calcium plays many important roles. It is vital in controlling the constriction (closing) and relaxation (opening) of your blood vessels. It also aids proper nerve impulses, muscle contraction, and release of your hormones. Calcium is a key factor for good recovery from any surgery involving your bones. Only about 30 percent of the calcium in your food is actually absorbed in your digestive tract. You lose a certain amount of calcium in your urine every day. This depends on how much caffeine you drink. Too much or not enough protein in your diet will affect your calcium absorption and the strength of your bones. Calcium rich foods include: kale, sardines, yoghurt, broccoli, watercress, cheese, bok choy and almonds. Magnesium – essential nutrition for recovery after surgery Magnesium plays important roles in the structure and the function of the human body. Over 60 percent of all the magnesium in your body is found in your bones. About 27 percent is found in muscle, while six to seven percent is found in other cells. Magnesium is required by many other nutrients, like vitamin D and calcium, to function properly. Proper wound healing after surgery requires the right amounts of calcium and magnesium in the fluid around the cells involved in the injury. If you are having surgery that involves your bones, magnesium is very important for the best outcome. Magnesium rich foods include: spinach, dark chocolate, pumpkin seeds, almonds, black beans, avocado, yoghurt and banana. Zinc essential nutrition for surgical recovery Zinc is an essential trace element for all forms of life. Zinc deficiency has recently been recognized by a number of experts as an important public health issue.

Vitamin D deficiency: Should we use supplements?

For years, it has been widely accepted by scientists that vitamin D deficiency is a longstanding health problem, with implications on health and wellbeing. While there are a wealth of reasons for vitamin D deficiently, inadequate skin synthesis due to restricted sunlight, limited food fortification and lack of vitamin D from natural food sources are confirmed contributing factors. Here, we discuss how certain health conditions can further enhance the risk of vitamin D deficiency and ways to increase its consumption. The levels of vitamin D deficiency In the UK, currently, Reference Nutrient Intakes (RNI) for vitamin D are only set for children aged 1.5-3 and adults over 65 years of age. Using the average intakes for each of these age groups, data from the latest UK National Diet and Nutrition Survey (NDNS) revealed that intakes of vitamin D from food sources are just 27% of the RNI for children and 33% for adults, showing significantly low vitamin D levels from food sources. When it comes to assessing blood biomarkers in relation to vitamin D levels, plasma vitamin D levels of <25 nmol/L are used to identify a deficiency. Using this as an indicator, it was revealed that on average, 19.7% of boys and 24.4% of girls aged 4 to 10 had levels less than 25 nmol/L, while 24% of men and 21.7% of women aged 19 to 64 has plasma vitamin D levels below this. The survey also revealed that when averaged out across the year, 40% of young people and adults were deficient between the months of January and March. Health conditions which can increase the risk of vitamin D deficiency Malabsorption, coeliac disease, cystic fibrosis, short bowel syndrome and inflammatory bowel disease (IBD) can all increase the risk of vitamin D deficiency. Amongst adults, an Irish study involving 81 patients with Crohn’s disease (CD) revealed that 63% were vitamin D deficient. Furthermore, a study of patients with coeliac disease in the US showed that 25% had vitamin D deficiency. Common signs of deficiency When it comes to symptoms of vitamin D deficiency, these tend to vary across the lifespan and with the level of deficiency. During adolescence, symptoms typical of severe deficiency include muscle cramps, pain in weight-bearing joints and problems walking, running or climbing stairs. In adults, a mild lack of vitamin D may present itself as general aches and pain, while more severe deficiency is associated with osteomalacia and post-menopausal increased fracture risk. Furthermore, suboptimal vitamin D levels are associated with an increased risk of severe infections and sepsis amongst those that re critically ill. Vitamin D supplements When it comes to supplementing vitamin D levels, we must first ascertain whether it is to maintain and prevent deficiency or to treat a diagnosed deficiency. In the care of maintaining levels, The European Food Safety Authority (EFSA) proposed an adequate intake of 15 µg/day for adults. In the UK, while RNIs are not available, it has been suggested that 10 µg/day for those aged 11 to 65 is suitable. In the situation of vitamin D deficiency, such as in cases of patients with diagnosed bone diseases, it has been advised by the National Osteoporosis Society that levels are increased rapidly with doses of up to 7500 µg. So, for example, taking 20 µg/day capsules, five times a day for 10 weeks could be used to rapidly correct low levels, and following this, levels should be maintained. When it comes to which form of vitamin D supplementation to take, evidence suggests that vitamin D3 (cholecalciferol) is effective at raising levels. Here at 121 Dietitian we support all the evidence regarding Vitamin D deficiency and the importance of correcting low levels. However the safety of increasing levels and continued maintenance of these levels are of concern, and so we prefer to guide our patients with a safe combination of a whole food diet rich in Vitamin D foods and natural supplementation of key nutrients. Why? Many of the general population are correctly taking action and looking after their own health, and over the counter supplementation has been around for a long time, nothing new there. The issue is the toxic levels of Vitamin D supplementation that people are taking and the length of time they are taking them for with no monitoring. Hypervitaminosis D is a rare but potentially serious condition. It occurs when you take in too much vitamin D. It’s usually the result of taking high-dose vitamin D supplements without checking your status. Too much vitamin D can cause abnormally high levels of calcium in the blood. This can affect bones, tissues, and other organs. It can lead to high blood pressure, bone loss, and kidney damage without treatment. Vitamin D can also affect Vitamin A, Vitamin K and magnesium levels and if the ratio is not maintained then you may experience further unpleasant side effects. As of the past number of years GP’s in Northern Ireland are unable to routinely measure peoples Vitamin D levels.  This for me as a health professional is disappointing and worrying as we are frequently seeing cases of severe deficiency and many with long term excessive use and negative consequences. Monitoring needs to happen. In summary, Vitamin D deficiency is common in the UK, particularly during the winter months and notably in vulnerable populations. While symptoms can be hard to spot, tiredness and general fatigue are common and may suggest your levels should be assessed by your GP or through a simple blood test where possible.
